Target Audience: 
State and local agency personnel working in the Breast & Cervical Cancer program
Course Length: 2 days
Course Description:  
This training will provide a “hands-on” approach to the Healthworks/DCFS functions within the Cornerstone software system. The course includes: Overview of the Healthworks Program, Documentation of DCFS Ward Data, Medical Care Plan Development, Assessment and Case Notes.
Performance Objectives:
Upon completion of the course, participants will be able to:

• Demonstrate proficiency in enrolling a new client on the
  Cornerstone system
• Effectively complete the processes required to enroll a client into the Healthworks Program
• Complete Family Case Management activities as they relate to DCFS Wards and the Healthworks program (assessments, medical information, and case notes)
• Complete a Health Works Care Plan, placing emphasis on medical issues
