Cornerstone Training
HEALTHY FAMILIES ILLINOIS
Target Audience:
State and local agency personnel working with the Healthy Families Illinois
program (HFI) Program.
Course Length: 2 days
Course Description:
This training will provide a hands-on approach to the Healthy Families
Illinois functions within the Cornerstone software system. The course includes: Client Enrollment, Completion of Family Stress Checklist, Healthy
Families Care Plan Development, and Recording Time & Activity Logs.
Performance Objectives:
Upon completion of the course, participants will be able to:
- Effectively complete the processes required to enroll a client into HFI
- Identify processes required for assigning a case manager within HFI
- Record information obtained from the Family Stress Checklist into
Cornerstone
- Complete an HFI Care Plan
- Document and record information on the Time and Activity Entry
screens

This activity has been submitted to WNA CEAP for approval to award contact hours. Wisconsin Nurses Association is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.
