Cornerstone Training
HEALTHY FAMILIES ILLINOIS

Target Audience: 
State and local agency personnel working with the Healthy Families Illinois program (HFI) Program.
Course Length: 2 days
Course Description:  
This training will provide a “hands-on” approach to the Healthy Families Illinois functions within the Cornerstone software system. The course includes: Client Enrollment, Completion of Family Stress Checklist, Healthy Families Care Plan Development, and Recording Time & Activity Logs.
Performance Objectives:
Upon completion of the course, participants will be able to:

  • Describe how to complete the processes required to enroll a client into HFI.
  • Discuss how to complete program assessments.
  • Develop a participant care plan.
  • Enter participant health data into the Cornerstone system.
  • Describe the upload/download process of Cornerstone laptops
  • Demonstrate how to use various screens in the Cornerstone system.
  • Explain how to document case notes on the clients enrolled.
  • Determine participant eligibility.
  • Describe how to complete the HFI enrollment process.
  • Identify proper procedures when terminating a client.

This activity has been submitted to WNA CEAP for approval to award contact hours. Wisconsin Nurses Association is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.
