Program Training 
FAMILY CASE MANAGEMENT

Target Audience: 
State and local agency personnel working as Case Managers with the Family Case Management Program.
Course Length: 1-2 days plus a 2-3 hour online course
Course Description:  
Staff registering for this training must complete a 2-3 hour web based training module prior to attending the additional one day training in either Chicago or Springfield. The web based and classroom training are designed to address the needs of new case managers and cover each step of the Case Management process: client identification and outreach; family assessment; service planning and resource identification; linking clients to needed services; service implementation and coordination; monitoring service delivery; advocacy; and evaluation. There will also be a 2nd day of training available for non-nurse case managers which will cover health issues related to pregnant women and infants.
Performance Objectives:
Upon completion of the course, participants will be able to:
  • Discuss the dual emphasis of the Case Manager
  • Identify target population and participant categories
  • Discuss how to encourage empowerment as a Case Manager
  • Describe the implementation/coordination role of the Case Manager
  • Identify ways to accomplish interagency cooperation at the State and local level
  • Describe strategies for advocacy at the client and system level
  • Describe the purpose of evaluation at both the client and system level
